Anthony L. Jordan Health Center is conveniently located off of Hudson Avenue at 82 Holland Street in Rochester, NY 14605. Our caring, compassionate staff is available to assist you with your healthcare needs through a wide variety of services.

For information on any of our departments and services or to schedule a same day/next day appointment, please contact us at (585) 423-5800. Home to our Women, Infants and Children (WIC) Supplemental Nutrition program, Jordan Health Link is located at 273 Upper Falls Boulevard, where you can also take advantage of non-emergent medical and dental services.
Contact Health Link at (585) 454-2630 to set up your WIC appointment and same day medical/dental visit. Both facilities are conveniently located along regular bus routes. Bus #4 stops in front of the main health center at 82 Holland Street. Bus # 7 stops next to Jordan Health Link at Tops Plaza.
