What should I do in case of emergency?
If patients have an emergency that cannot wait until the following business day, they should go to the nearest emergency room, immediately. For non-urgent care, they can call for an appointment the next morning.
What is your privacy policy?
Our policy is to provide confidential healthcare. What the patient says stays between him/her and the provider. However, in the best interests of the patient, if he or she is in danger, we will let the proper people know what is happening.
How can I prepare for my appointment?
Patients should bring a list of any medications they are taking, as well as their insurance cards when they come to the HIV Prevention & Primary Care Department.

Services
A wide range of problems are treated at Jordan, from depression to family stress to coping with chronic illness. At the behaviorial health department, clients consistently find the support, respect, and guidance they need to make it through difficult times and create a brighter future for themselves and their families.
Hepatitis C
Jordan provides primary care, care coordination, and extensive peer support services for patients diagnosed with Hepatitis C. Patients in this program are eligible for the latest medical treatment protocols that could slow the progression of the disease. The New York State grant also provides a special insurance pool to Hepatitis C diagnosed patients, who otherwise cannot afford this treatment.
Services include HCV testing in a caring and confidential environment and emotional support provided by professional counselors who are on-site to help patients understand the test and the results. The Center also offers treatment services and medications to help patients who are Hep C positive through management and treatment of their disease. We offer counseling, mental health services, chemical dependency services, care coordination, healthcare planning, family services, and support groups to address the complex array of issues associated with diagnosis and treatment. Patients receive education on the virus and its effects on the community and the individual, how it is transmitted, and how to protect themselves and others to minimize transmission. Treatment is not for everyone but knowing your options is a way to start.
Human Immunodeficiency Virus (HIV)
HIV is a complicated illness that requires specialized treatment. HIV testing and treatment is provided by a team of caring professionals in a safe and strictly confidential environment.
At AJHC, we believe everyone should know their HIV status. The HIV test is quick and easy and results can take as little as 10 minutes. Know your status. It is the first step in keeping yourself healthy. After your test, ask about other things you can do to keep yourself healthy.
If you are HIV positive, treatment is provided by a skilled physician with over 20 years of experience in the field of HIV/AIDS. This physician and PPC nurses work closely with each patient to develop a treatment that reduces the harmful effects of HIV. The PPC medical team will work with you to develop medication and treatment regimens tailored to your unique health conditions and needs. Follow up care ensures the best health is maintained. A full range of medical and dental care is available on site. AJHC can be your complete medical home.
All patients diagnosed with HIV receive medical care coordination services. Dedicated and supportive care coordinators assist patients in understanding the HIV disease, managing their appointments, learning ways to remain healthy and how to protect others from transmission. Patients can also receive assistance with transportation and referrals to outside agencies. Care Coordinators frequently provide emotional support and encouragement for patients coping with the virus.
Subaxone
The Center is happy to provide buprenorphine treatment to individuals for opiate dependence as part of our behavioral health program. This includes people addicted to heroin and narcotic medications like Oxycontin and Vicodin. There are currently three doctors certified to provide buprenorphine (Suboxone® or Subutex®) We screen individuals to make sure that they are appropriate candidates for this program. We support the recovery and sobriety of our patients by having a dedicated coordinator for our buprenorphine prescribing program, and a mental health counselor (licensed social worker) as part of this program.
