Las leyes estatales y federales nos obligan a mantener la confidencialidad de su información de salud y para informarle sobre nuestras prácticas de privacidad al proporcionarle este aviso.  Debemos seguir las prácticas de privacidad tal como se describe a continuación.  Podemos cambiar nuestras prácticas de privacidad. Si cambiamos nuestras prácticas de privacidad que el cambio se aplicará a toda su información de salud que tenemos. Si cambiamos nuestras prácticas de privacidad publicaremos los cambios en cada uno de nuestros centros y le proporcionaremos una copia en su próxima cita. USOS Y DIVULGACIONES DE INFORMACIÓN DE MÉDICA

Le mantendremos su información médica confidencial, solo será utilizando únicamente para los fines siguientes:

Tratamiento: Nosotros podríamos usar su información médica para ofrecerle nuestros servicios profesionales.  Hemos establecido estándares “mínimo necesario ó necesita saber” que limitan el acceso de varios empleados a su información médica de acuerdo a sus funciones laboral.  Cada uno de nuestros empleados  firma una declaración de confidencialidad.

Desglosar: Nosotros podemos revelar y / o compartir su información médica con otros profesionales médicos que proveen tratamiento y / o servicios.  Estos profesionales médicos tienen una política de privacidad y confidencialidad como éste.  Información médica sobre usted también puede ser divulgada a su familia, amigos y otras personas que usted elija como parte de su atención médica, sólo con su autorización.

Pagos:   Nosotros podemos utilizar y divulgar su información para obtener pagos por los servicios que se le ha brindado.  Esto implica, que nuestro personal de Facturación Médica  u otras organizaciones de seguros o colectores, intervengan en el proceso de facturas por correo para  colectar el balance sin cobrar o  los saldos pendientes.

Emergencias:  Nosotros podemos usar o divulgar su información médica, o ayudar al momento para notificar a un miembro de la familia o a los responsables de su cuidado, en caso de cualquier emergencia que implique de su atención, su ubicación, su condición general o muerte.  De ser posible, le daremos la oportunidad de oponerse a este asunto o divulgar.  Bajo condiciones de emergencias o si está incapacitado, usaremos nuestro juicio profesional para revelar únicamente dicha información  necesaria directamente a usted.  También utilizaremos nuestro juicio profesional al momento de tomar una decisión  respecto a recetas médicas, rayos-x, u otras formas similares de información médica y suministros a menos que nos han aconsejado lo contrario.

Operaciones de cuidados de salud: Nosotros podemos usar o divulgar su información médica para mantener nuestra práctica operable.  Ejemplos de personal, que pueda tener acceso a esta información incluyen, sin limitaciones a, nuestro personal médico, otros centros de salud, personal administrativo y personas que realizan actividades similares.

Requerido por la Ley: Nosotros podemos usar o revelar información médica cuando es requerido por la ley (Tribunal u órdenes administrativas, citación, solicitud de descubrimiento u otro proceso legal).  Esta información se usara ó divulgara cuando sea requerida por la seguridad nacional, de inteligencia y divulgar su información cuando solicitada por seguridad nacional, inteligencia y otro Estado y funcionarios federales y si usted es un recluso o no bajo la custodia del cumplimiento de la ley.

Abuso ó Negligencia: Nosotros podemos revelar su información médica a las autoridades correspondientes si consideramos necesaria que usted es posible víctima de abuso, negligencia o violencia doméstica o posible victima de otro crimen. Esta información se revelará únicamente en la medida necesaria para impedir una amenaza seria a su salud o la seguridad o de los demás.

Responsabilidades de Salud Pública: Revelaremos su información médica para informar de problemas con productos, reacciones a medicamentos, producto recuerda, expuestas a enfermedades y infección, lesiones y/o discapacidad.

Servicios de mercadeo relacionadas con la salud: Nosotros no podemos utilizar su información médica para propósito de propaganda o promoción, al menos que usted nos dé la autorización por escrito.  Esta decisión puede ser retirada en cualquier momento y nosotros dejaremos de utilizar su información médica.

Seguridad Nacional: Su información de salud podría ser desglosada a las autoridades militares de las Fuerza Armas en cualquier momento necesario.  Si su información es requerida por Inteligencia Lícita, Contrainteligencia u otras agencias nacionales, nosotros podemos desglosar su información médica a funcionarios federales autorizados.

Recordatorios de Cita Médicas: Nosotros podemos divulgar su información médica para proveerle a usted con recordatorios de cita médicas, información acerca de otros tipos de tratamiento y otros servicios que le pueden interesar. Nos podemos contactar tanto por teléfono, dejando un mensaje de voz, por correo electrónico, por mensaje de texto, enviándole postales o cartas o por otros medios.

Recaudación de Fondos: Nosotros podemos usar y / o revelar alguna de su información de salud con el fin de contactarle para actividades de recaudación de fondos para beneficios del Centro. Si le enviamos materiales para recaudar fondos, los materiales describirán cómo usted puede pedir que dejemos de enviarle materiales para recaudar fondos.

Otros usos y desgloses: Nosotros NO utilizaremos o divulgaremos su información de salud por cualquier otro motivo sin su permiso escrito. Si usted nos da permiso para utilizar o revelar información sobre su salud por razones distintas de las que hemos enumerado aquí, usted puede retirar su consentimiento en cualquier momento y nos dejará de usar su información de salud por esas razones cuando usted retiré su permiso.

SUS DERECHOS DE PRIVACIDAD COMO NUESTRO PACIENTE

Comunicación Confidencial: Usted tiene derecho a recibir comunicación confidencial de su información médica de una manera y a una localidad que razonablemente se solicite. Puede hacer su solicitud por escrito a nuestro Oficial de Privacidad.

Acceso: Con previa solicitud escrita, usted tiene derecho a revisar y obtener copias de su expediente médico (y a los individuos que sean tutores legales).  Habrá algunas excepciones limitadas.  Si desea revisar su expediente médico, será necesario completar y enviar un Formulario de Solicitud correspondiente.  Para obtener una copia del Formulario de Solicitud, deberá contactarse con nuestro Departamento de Expedientes Médicos.  También puede solicitar acceso enviando una carta a la dirección al final de este aviso.  Una vez aprobado, puede hacer una cita para revisar sus documentos.  Copias, solicitadas costarán $.75 para cada página.  Si desea que las copias sean enviada por correo a usted, tendrá un cargo adicional.  Si prefiere un resumen o una explicación de su información de salud, mas detallado, favor de comunicarse con nuestro Oficial de Privacidad.

Enmienda: Usted tienen el derecho a modificar su información de salud, si usted siente que es inexacta ó incompleta.  Su solicitud debe ser por escrito y debe incluir una explicación del por qué debería modificarse la información.  En circunstancias determinadas, su solicitud puede ser negada.

Desglose No-Rutinaria: Usted tiene derecho a recibir una lista de desglose no-rutinaria que hemos preparado de su información médica (cuando hacemos un desglose de rutina de su información a un profesional de tratamiento y/ó propósito de pago, no mantenemos un registro de accesos rutinarios, por lo tanto, no están disponibles).  Usted tiene el derecho a una lista de casos en los que nosotros, o nuestros socios de negocios, a conocer la información por razones que no sean para tratamiento, pago, operaciones de cuidado de salud, cuando usted nos ha dado permiso para compartir su información de salud, u otras instancias en que están permitidos por ley a revelar su información médica.

Restricciones: Usted tiene derecho a solicitar restricciones adicionales en nuestro uso o divulgación de su información médica. No estamos obligados en aceptar esas restricciones adicionales, pero si lo hacemos, cumpliremos con nuestro acuerdo, excepto en situaciones de emergencia.  Favor de contactar a nuestro Oficial de Privacidad de usted  restringir aún más el acceso a su información médica.  Esta petición deberá ser por escrito.

Copia de Notificación: Usted tiene derecho a recibir una copia impresa de este aviso de parte de nosotros cuando usted lo solicite.

PREGUNTAS Y QUEJAS

Usted tiene el derecho de presentar una queja con nosotros si usted siente que no hemos cumplido con nuestras políticas de privacidad o que hemos violado sus derechos de privacidad, o si está en desacuerdo con una decisión que tomamos sobre el acceso a su información de salud. THIS NOTICE DESCRIBES HOW HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED
AND HOW YOU CAN GET ACCESS TO THIS INFORMATION.

PLEASE REVIEW IT CAREFULLY.

State and Federal laws require us to maintain the privacy of your health information and to inform you about our privacy practices by providing you with this notice.  We must follow the privacy practices as described below.  We can change our privacy practices. If we change our privacy practices the change will apply to all of your health information that we have. If we change our privacy practices we will post the changes in each of our centers and provide you with a copy when you come in for an appointment. You may request a copy of our Privacy Notice at any time by contacting our Medical Record Department at (585) 423-5885.

TYPICAL USES AND DISCLOSURES OF HEALTH INFORMATION

We will keep your health information confidential, using it only for the following purposes:

Treatment:  We may use your health information to provide you with our professional services.  We have established “minimum necessary or need to know” standards that limit various staff member’s access to your health information according to their primary job functions.  Everyone on our staff is required to sign a confidentiality statement.

Disclosure:  We may disclose and/or share your healthcare information with other health care professionals who provide treatment and/or services to you.  These professionals will have a privacy and confidentiality policy like this one.  Health information about you may also be disclosed to your family, friends, and/or other persons you choose to involve in your care, only if you agree that we may do so.

Payment:   We may use and disclose your information to seek payment for services we provide to you.  This disclosure involves our business office staff and may include insurance organizations or other businesses that may become involved in the process of mailing statements and/or collecting unpaid balances.

Emergencies:  We may use or disclose your health information to notify, or assist in the notification of a family member or anyone responsible for your care, in case of any emergency involving your care, your location, your general condition or death.  If at all possible, we will provide you with an opportunity to object to this use or disclosure.  Under emergency conditions or if you are incapacitated, we will use our professional judgment to disclose only that information directly relevant to your care.  We will also use our professional judgment to make reasonable inferences of your best interest by allowing someone to pick up filled prescriptions, x-ray or other similar forms of health information and/or supplies unless you have advised us otherwise.

Health Care Operations:  We may use or disclose your health information to keep our practice operable.  Examples of personnel who may have access to this information include, but not limited to, our medical staff, outside health or management reviewers and individuals performing similar activities.

Required by Law:  We may use or disclose your health information when we are required by law to do so  (Court or administrative orders, subpoena, discovery request or other lawful process).  We will use and disclose your information when requested by national security, intelligence and other State and Federal officials and/or if you are an inmate or otherwise under the custody of law enforcement.

Abuse or Neglect:  We may disclose your health information to appropriate authorities if we reasonably believe that you are a possible victim of abuse, neglect or domestic violence or the possible victim of other crimes.  This information will be disclosed only to the extent necessary to prevent a serious threat to your health or safety or that of others.

Public Health Responsibilities:  We will disclose your health information to report problems with products, reactions to medications, product recalls, disease/infection exposure and to prevent and control disease, injury and/or disability.

Marketing Health Related Services:  We will not use your health information for marking purposes unless we have your written authorization to do so. If you give us permission to use your health information for marketing purposes, you may withdraw your permission at any time and we will stop using your health information for marketing when you withdraw your permission.

National Security:  The health information of Armed Forces personnel may be disclosed to military authorities under certain circumstances.  If the information is requested by lawful intelligence, counterintelligence or other national security activities, we may disclose it to authorized federal officials.

Appointment Reminders:  We may disclose your health information to provide you with appointment reminders, information about other types of treatment and about other health services we think you might be interested in. We may contact you by telephone, by leaving voicemail messages for you, by e-mail, by text message, by sending you postcards or letters, or by other methods.

Fundraising: We may use and/or disclose some of your health information in order to contact you for fundraising activities supportive of the Center.  If we send you fundraising materials, the materials will describe how you can ask us to stop sending you fundraising materials.

Other Uses and Disclosures: We will not use or disclose your health information for any other reason without your written permission. If you give us permission to use or disclose your health information for reasons other than those we have listed here, you may withdraw your permission at any time and we will stop using your health information for those other reasons when you withdraw your permission.

YOUR PRIVACY RIGHTS AS OUR PATIENT

Confidential Communications: You have the right to receive confidential communication of your health information in a way and to a location as you reasonably request. You may make your request in writing to our Privacy Officer.

Access: Upon written request, you have the right to inspect and get copies of your health information (and that of an individual for whom you are a legal guardian). There will be some limited exceptions. If you wish to examine your health information, you will need to complete and submit an appropriate request form. Contact our Medical Records department for a copy of the Request Form.  You may also request access by sending a letter to the address at the end of this Notice. Once approved, an appointment can be made to review your records. Copies, if requested will cost $.75 for each page. If you want the copies mailed to you, postage will also be charged. If you prefer a summary or an explanation of your health information, we will provide it. Please contact our Privacy Officer for an explanation of our structure.

Amendment:  You have the right to amend your healthcare information, if you feel it is inaccurate or incomplete.  Your request must be in writing and must include an explanation of why the information should be amended.  Under certain circumstances, your request may be denied.

Non-routine Disclosure:  You have the right to receive a list of non-routine disclosures we have made of your health care information (when we make a routine disclosure of your information to a professional for treatment and/or payment purpose, we do not keep a record of routine disclosures, therefore these are not available).  You have the right to a list of instances in which we, or our business associates, disclosed information for reasons other than for treatment, payment, heath care operations, when you have given us permission to share your health information, or other instances when we are permitted by law to disclose your health care information.

Restrictions:  You have the right to request that we place additional restrictions on our use or disclosure of your health information.  We do not have to agree to those additional restrictions, but if we do, we will abide by our agreement except in emergencies.  Please contact our Privacy Officer if you want to further restrict access to your health care information.  This request must be in writing.

Copy of Notice: You have the right to receive a paper copy of this notice from us when you request it.

QUESTIONS AND COMPLAINTS

You have the right to file a complaint with us if you feel that we have not complied with our Privacy policies or we have violated your privacy rights, or if you disagree with a decision we make regarding access to your health information.  You may file a complaint with in writing with our Privacy Officer.   You can get a Complaint Form from our Privacy Officer. You may also file a complaint with the U.S. Department of Health and Human Services if you feel that we have violated your privacy rights. We support your right to privacy of your information and will not retaliate in any way if you choose to file a complaint with us or with the U.S. Department of Health and Human Services.

Jordan Health Center Board Appoints Interim CEO
Rochester, NY, October 15, 2012 – Anthony L. Jordan Health Center (AJHC) corporation board chair Dr. Buddhi Shrestha announced today that its dental director, Dr. Janice Harbin Joseph, has been appointed interim CEO, effective November 1, 2012. He also announced that Jordan Health Center will soon commence a local and regional executive search, to recruit the successor for the current Health Center CEO Dr. Bridgette Wiefling, who last week tendered her resignation to join Rochester General Health System.
“Dr. Wiefling turned Jordan Health Center around and we are all happy for her new opportunity. We are very pleased that Dr. Harbin Joseph has agreed to accept the position of Jordan’s interim CEO and we expect to stay the course,” Shrestha said. In her current position as dental director overseeing dentistry at all of Jordan’s sites, Dr. Harbin Joseph has highlighted good dental practices for children and adults, and increased patient visits.
Dr. Harbin Joseph joined Jordan Health Center earlier this year. She came to the Center from Detroit, MI bringing more than 30 years of clinical and leadership experience with a primary focus in Public Health. Shrestha informed the staff of the continuity of leadership, noting: “As a public health advocate, Dr. Harbin Joseph believes in Jordan’s mission; as a provider, she understands the needs of our patients, and as a leader she understands the vision and direction of our Health Center.”
Prior to joining AJHC, Dr. Harbin Joseph headed the Adult Dental Department for the Detroit Department of Health and Wellness Promotion. From 2003 to 2009, Harbin Joseph served as Dental Director for Detroit Health Care for the Homeless. She is the Immediate Past Chair of the Michigan Oral Health Coalition Board of Directors, serving for eight years as a board member. In 1999, Dr. Harbin Joseph worked on the President’s Council on Sustainable America with Vice President Al Gore.

Dr. Harbin Joseph is an active member of the American Dental Association, National Dental Association, Michigan Dental Association, Detroit District Dental Society, Wolverine Dental Association, and the Michigan Oral Health Coalition. Past awards include the Distinguished Service Award from Michigan Primary Care Association, the Champion for the Underserved Award from Voices of Detroit Initiative, and the Promoting Oral Health Award from Colgate Dental Health Education Advisory Board. Dr. Harbin Joseph holds a Doctorate of Dental Surgery degree from Howard University College of Dentistry.

In addition to her strengths in dentistry and healthcare leadership, Dr. Harbin Joseph honed her community organizing skills with Metropolitan Organizing Strategy Enabling Strength (MOSES), a grassroots initiative working for social justice across southeast Michigan. Dr. Harbin Joseph worked with city and suburban, Christian, Jewish and Muslim faith-based organizations, and many cultural institutions, which provides her with a unique perspective and sensitivity when working with the population served by Jordan Health Center.

“As a healthcare professional well-acquainted with the role Federally Qualified Health Centers play in the community, I am honored to have this opportunity,” Harbin Joseph said. “I look forward to continuing the great work of the Center with Jordan’s board and staff during this transition period,” she said, adding: “the journey continues for Jordan.”
