Anthony L. Jordan Health Center is a not-for-profit comprehensive primary care center in Rochester NY, widely recognized for being one of the first true community health centers in the country. Located in the northeast section of the City of Rochester where health disparities are the greatest, Jordan collaborates with key providers, local schools and other neighborhood-based assets to facilitate increased access to care. We celebrate these collaborations and our achievements each year during our Annual Luncheon. In 2012, we gratefully honored Elliott Landsman with the President’s Award and Steve Goldstein was recognized with our award for Lifetime Achievement.

Moving forward efficiently
Like our solid concrete building, Jordan’s services are built to last through turbulent times and our loyal, mission-driven Board members and staff continue to put “Patients First.” Our unwavering goal remains simply this: to provide high-quality care with dignity and respect to individuals of all ages regardless of their ability to pay. Recent advances toward our goal include complete transition to an electronic medical records system (EMR), Lean 6 Sigma process optimization, and achieving the Level III Patient Centered Medical Home designation. Independent and affiliated
Jordan Health Center is an independent, federally qualified community health center with a Level III Patient Centered Medical Home designation through the National Committee on Quality Assurance. We receive funding from the Health Resources and Services Administration of the Department of Health and Human Services to provide sliding fee scale discount primary care services to medically underserved and uninsured patients.

The Health Center provides certain programs and services in conjunction with the University of Rochester Medical Center and its related hospitals, Strong Memorial Hospital and Highland Hospital.
